This is a 'tipless' nitinol stone basket design to pass through felxible ureteroscopes and cystoscopes. The unusual feature of this device is the ability to deflect the tip away from the midline. This is thought to help grasping stones directly in front of the basket.

 

Features

This is a 'tipless' nitinol stone basket design to pass through felxible ureteroscopes and cystoscopes. The unusual feature of this device is the ability to deflect the tip away from the midline. This is thought to help grasping stones directly in front of the basket.

 

Design/construction

The basket is constructed from Nitinol attached to two stainless steel wires that allow basket movement. the handle has a thumb slide to retract and close the basket and a wheel that turns to move the basket tip. The handle can be dimantled to allow scope removal over the wire if the stone becomes impacted.

The basket is available in 10,13 and 16mm sizes and is 3F.

In Use

The basket operation is the same as all other baskets until you turn the wheel on the handle. It shifts the point of union of the wires off to one side allowing the basket to be pushed onto the stone. It also slightly deviates the basked to one side giving you a bit more angulation, although this effect is not as great as I would have liked.

A great feature is the ability to torque the basket by turning the handle. this is something I have found to be lacking in other baskets I have used.

Results so far

I have only limited experience with two patients so far. One of which is shown in the review images. The basket allowed us to 'catch' this stone that was partially hidden just over the lip of the infundibulum. We then pulled it back and worked on it with the US lithotripsy through a rigid scope. The alternative would have been to use a 'Goose-neck' snare.

Comparison with ?

My main comparison is with the Boston Scientific 'zero tip' nitinol basket. This is similar but does not have the tip deflection capability and does not seem to be torqueable.

The advantage of the Boston basket is its smaller size (2.4F) for the 12mm basket. this allows greater room down the flexible scope's working channel for a laser fibre and allows improved irrigation.

Conclusion

A useful addition to aid stone clearance.
