The Trigen Sureshot System is a computerised tracking system which facilitates the placement of the distal locking screws after intramedullary nailing using the Trigen nail.  Distal locking is a challenge which most trainees take a few cases to master, but although it may become easier with practice, it requires radiation exposure as a matter of course. The Sureshot system uses an electromagnetic technology to establish the correct location and alignment for distal locking, and Smith and Nephew have data to show that this reduces operating time and radiation exposure.

During a demonstration, the device appeared to perform as advertised. The targeting sleeve is set in the centre of a reasonably heavy “pram wheel”, and it would be reasonable to expect to have an assistant holding it whilst drilling the hole. The sensor rod in the centre of the nail is single use and drives the cost of using the device. The display was easy to interpret, and would likely be straightforward to use in the operating theatre. As it is an electromagnetic system, it is susceptible to interference from large bits of metal – notably the clamp used in the demonstration, but this seems unlikely to be a problem in the operating theatre.
