Having done two of these in a week I thought it was time to make another video of the technique. This is essentially a PTC but with access into the portal vein. The islets are then infused under gravity and they settle in the liver and produce insulin. The real science is in the preparation of the islet suspension to make them suitable for infusion. This 18 minute video will take you through the process of PTC and stent insertion from local anaesthetic all the way through to plugging the tract with gelfoam at the end of the procedure. Only consider plugging if you are confident that there is good drainage and the patient is not septic. Otherwise leave in an external drainage tube as a minimum or internal/external drain so that access is maintained for further dilatation or stenting if required.
