The Vanguard Complete Knee System from Biomet is a comprehensive suite of implants which include primary cruciate retaining and posterior stabilised systems (cemented and uncemented). Tibial components include the Monobloc system which has a one-piece moulded tibial tray (compare with the AGC knee) and a modular tibial system which can support a cruciate retaining tray, a cruciate retaining lipped tibial tray, an anterior stabilised tibial tray with a more prominent anterior lip, and a posterior stabilised insert, which has a rounded post. Tibial trays have either a finned or an I-beam tibial stem.

Increasing constraint variants, including the posterior stabilised plus tray, the SSK (Super Stabilised Knee) PS tray, and the SSK PSC tray, which have posts of increasing size are available as part of the revision system. High flexion and rotating hinge versions are also available, as is a patellofemoral replacement. Customised jigs (Signature Knee Instrumentation) are available for the primary knee system.

The tibial component is manufactured from ArCom polyethylene but an E1 polyethylene version (vitamin E infused) is also available.

Features of the system include 10 femoral sizes, a relatively deeper and longer trochlear groove for patellar support in flexion, a narrow frontal profile to reduce the risk of femoral overhang and fully interchangeable sizes.

In the England and Wales National Joint Registry 9th Annual report, the Vanguard knee had 6% of the market and a 2.2% (1.6% to 2.9%) revision rate at 3 years (MRK was lowest at 1.14% (0.8% to 1.6%), and Columbus highest at 2.95% (2.14% to 4.07%) in the report).
