I first came across the KnifeLight during my fellowship in Singapore in 2001 where the consultants were using it. The device is simple, elegant and idiot-proof to use. It enables the surgeon to perform carpal tunnel release in less than 10 minutes through a mini incision of 1cm, without jeopardising the important structures in the area. The concept of illumination is brilliant (pun intended) and allows the surgeon to ensure complete release of the transverse carpal ligament.
Attempts have been made to replicate the device and improve upon it, but no success so far has been reported. Comparing the Stryker Knife with a baby Metzenbaum (Noir by Aesculap), I find the former smoother to use (of course it's a sharp blade everytime!), a definite end point is felt and the illumination helps in determining complete release. I find post-op pillar pain less of an issue as well.
