I use the AP because I think it makes sense to use a third-generation prosthesis, of which a number of good versions are on the market. I tend to perform TSR for osteoarthritis in patients over the age of around 70, as I think it provides better pain relief that hemiarthroplasty, and the revision rate is low in this group.
The AP is daunting to users and theatre staff initially due to the tower that is used to assemble the humeral component. The head fit and angle is determined intraoperatively on a ball adaptor between the stem and head, tightened, removed, and the tower is used to replicate this assembly in the definitive prosthesis. This is relatively straightforward, however, and usually I have found the fixed 135 degree setup to be applicable, it which the only variable is the eccentric head.
The instrumentation is generally easy to use, and postoperative xrays usually demonstrate good replication of anatomy. I expect the long term results to be comparable to other similar implants.
Personally I have found the company support excellent.
