Comparative Data: No randomized trials directly compare Indigo/Lightning to other thrombectomy devices (e.g., FlowTriever) or manual aspiration, limiting claims of superiority. EXTRACT-PE and STRIKE-PE lack control arms.
Sample Size and Scope: Studies like EXTRACT-PE (119 patients) and ALI series (18 patients) are modest, and Lightning-specific trials (e.g., Flash) are nascent or unpublished in full.
Outcome Metrics: Efficacy often focuses on technical success (flow restoration) and surrogate markers (RV/LV ratio), with less data on long-term patient outcomes like survival or quality of life.

Recently used the PE 12 F device for a life threatening acute PE where thrombolysis was not possible due to recent neurological procedure.
A long 14F sheath parked in the main pulm trunk is helpful. I found the catheter tip soft but torquable and could aspirate enough thrombus to make a difference to the patient. The ability of the device to tell whether it’s in free flow or in thrombus is very helpful. The thrombus separator is good at dislodging clot in a blocked Catheter.
