Whilst this is an excellent wire for peripheral angioplasty (and the 018 size can be your only wire that might need on the trolley) it can sometimes be hard to steer, hence the 4 rating,
Of note, it is also excellent in the urinary tract (which I think is an off label use).

I used this wire for a nephrostomy in a patient who found it difficult to keep still. Minimised changing wires and overall I found it easy to use... and will continue to do so!

A nice combination wire for stenting in the GI tract.
It is not as stiff as an Amplatz, but will just about guide a 28Fr delivery system for biodegradable oesophageal stents if used gently.
Excellent for colonic and particularly biliary stenting, as need for wire exchanges is reduced.

Disadvantages are:
1, Only available in an angled version, no straight tip
2, Hydrophilic coating degrades extremely quickly in gastric acid resulting in high friction within the catheter.

Glidewire Advantage Peripheral Guidewire : A good addition to the IR shelf. This is a combination wire with the distal 25cm being hydrophilic and a stiffer proximal body with PTFE coating. Comes in two lengths starting at 180cm.

Advantages : It is a single 0.35 wire that enables the operator to both cross the lesion and balloon angioplasty and deliver stents. This can therefore reduce flouro time and risk of vascular trauma or complication involved in multiple exchanges. Useful if one wants a stff body to track/push.
Specific useful sites -
a. Aortoiliac stenosis using high profile system with larger groin sheaths .
b. Venous stenosis inc fistula (venous side )

Disadvantages: Can be used only in limited situations (0.35 high profile system). The feel is no where close to a standard terumo.The maneuverability is limited. Can cross iliac moderate stenosis easily but it is not the same with severe stenosis following which a standard terumo crosses the severe stenosis like a piece of cake.

There are some uses for this wire in selected groups. Operators must try for themselves as the PTFE coating on the body has a different feel.
