This patient had a pancreas transplant removed and had a fistula into his bladder form the right external iliac artery. A Fluency stent was place 14 months ago with good result. he re presented with haematuria and rapidly dropping Hb requiring transfusion. You can see a new pseudoaneurysm arising from the top end of the old stent. I placed this Advanta V12 across its neck with good result. This balloon mounted stent is ideal as it can be accurately placed and only needs a 7F sheath. The fluency requires a 9F sheath which would have needed a perclose rather than starclose for haemostasis.
There is also an apparent AV fistula, possibly from a leash of vessels joining the common femoral vein ? I'm not sure if this is this inflammatory or a true AV fistula ? The CT showed significant enhancement in this region.
