This was an excellent opportunity to discuss the approach to primary knee arthroplasty with someone who has thought deeply about it. The general theme of restoring the pre-morbid alignment of the knee rather than the mechanical axis is one which is pertinent to the debate about patient specific instrumentation (see Stryker Shapematch)and which combined with minimal ligament releases may offer the patient a less traumatic route through knee replacement. The difference this makes to patients remains to be proven, but it likely makes sense for surgeons to be as atraumatic as possible as long as the implantation is not compromised.
