Sub-optimal drug eluting stent deployment can increase occurrence of late TVF and TVR. Post dilatation of stents with high pressure non compliant balloons can reduce restenosis by facilitating optimal stent deployment. The Hiryu is a non compliant balloon from Terumo for high pressure post dilation of coronary stents. It comes in a range of sizes (XXX) and lengths (YYY) is monorail, 5French catheter compatible with a hypertube design.

In use

I have used over 120 balloons alongside other high pressure dilation balloons. The Hiryu is a capable performer with excellent crossability and trackability. Even in difficult cases it can be delivered over “workhorse” wires. Once delivered to the target, it is an excellent balloon for post dilating and has a high burst rated pressure. The one problem we have encountered is the balloon bursting when taken beyond its nominal pressure – common practice when post dilating. Once deployed, its ability to re-cross is very good even in challenging cases.

Conclusion

The Terumo Hiryu non compliant balloon is a workhorse non compliant balloon with an excellent crossing profile. The ability of the balloon to be reform is excellent but is limited by a tendency to burst at pressures exceeding nominal.
