This seems to be a useful idea to enable you to advance a needle using a \'blunt\' central trocar when close to vital structures. I used it in this patient with retroperitoneal anterior pararenal collections from pancreatitis. The sharp stylet was first used through the needle to penetrate the superficial soft tissues. I then changed for the blunt trocar to advance the needle further to enter the collection. I then passed a wire through it and exchanged for a locking pigtail catheter.

I think in my practice I would find this very useful for performing hydrodissection around the kidney and bowel prior to RF or microwave ablation. The blunt trocar will make it less likely to damage bowel or other structures.
