Description

From Which Medical Device

The Low Contact Stress (LCS) knee system from DePuy is a rotating platform knee replacement system, first implanted in 1977.  The device has a polished cobalt-chrome tibial tray into which the peg of the polyethylene tibial component is placed to allow rotation. There are antero-posterior glide and rotating platform versions.

Other variants include cruciate-retaining and cruciate-substituting, as well as fixed and rotating platform patellar components, cemented and cementless (Porocoat) components. A hydroxyapatite coated uncemented version (DuoFix) is also available.

The geometry of the femoral component is such that the radius of curvature reduces during flexion, and the femoral component and tibial tray are designed to be maximally conforming. The reduced wear associated with the mobile bearing compared with fixed bearing designs is traded off against the risk of dislocation and polyethylene fracture, although these may be more likely with older versions of the AP glide design rather than the rotating platform (1)

The LCS Complete RPS knee system is the high-flexion variant.

The 9th National Joint Registry Report of England and Wales quoted a 2.39%(2.07% to 2.76%) 5 year revision rate for the LCS Complete knee. This compared to 1.14% for the MRK knee (the lowest) to 2.95% for the Columbus knee (the highest).
