With the newer iterations, the device is now much lower profile and sheath size has reduced (16F, 18F and 20F for 23mm, 26mm and 29mm Sapien XT respectively). In our institution, this has meant that around 80% of patients are suitable for a transfemoral approach.

Whilst no comparative randomised data exists regarding optimal access route, it is widely held that TF access is associated with shorter recovery times and is the least \"invasive\" of the available options.

As the device range spans annulus sizes from 18-27.5mm and access is possible through transfemoral, transapical and trans-aortic routes, there are very few patients who now have a technical contra-indication to TAVI with the Sapien XT system.
