CPCS stands for Collarless Polished Cemented Stem and is a very good example of a collarless polished tapered stem utilising the taper-slip philosophy popularised by the Exeter group. The ODEP 5A rated stem has standard and extended offset versions and a good range of sizes to accomodate virtually all anatomical variations. Templating is very helpful in obtaining the optimum fit.
The shoulder and proximal portions of the stem are somewhat narrower than similar tapered stems eg CPT & Exeter but this has never been a problem in practice and I am not awre of any reports of breakages.
Instrumentation is good, though depending on how aggressively you use the broaches initially I am aware of situations where they can get stuck in a tight femur. This is however technique dependent and there is a powered reamer to help reduce this risk.
Unlike the Exeter there are different sized centralisers which are checked using trials. Despite this it can be hard to get the sizes tight and the centralisers can appear undersized on the post-op films. This is one area where the Exeter is better.
Overall the prosthesis is relatively forgiving with good tolerance for slight over or under-sizing or varus/valgus malposition. Insertion is easy by standard techniques and outcomes appear very good. The tapers accept the Smith and Nephew cobalt chrome, Oxinium and ceramic heads. I have used this implant for over two years in combination with the Reflection cemented cup and overall am very happy with it.
